Jeff Sharkey de2b22fbc6 Handle pipe thumbnails, acquire unstable refs. ...

Support decoding thumbnails delivered over pipes by wrapping in a
buffered stream.  Also switch to using unstable provider references
to avoid crashing DocumentsUI.
